Pregnancy/Birth of Child Leave

Eligible mothers and fathers are entitled to up to 12 weeks of FMLA leave, if available, for the birth of their child and for bonding time to be with the newborn child during the 12-month period beginning on the date of the birth.

An eligible mother is entitled to FMLA for incapacity due to pregnancy, for prenatal care, or for her own serious health condition following the birth of the newborn child and/or for bonding time with the newborn.

An eligible father is entitled to FMLA beginning on the day of the child’s birth or for the medically documented incapacitation period of the spouse during pregnancy or recovery from labor and delivery, and/or for bonding time with the newborn child.

How to Apply

Submit BOTH items below via LeaveAdministration@UTSouthwestern.edu to Leave Administration for consideration of the FMLA request.

  1. A completed Family and Medical Leave Act Request Form (link requires VPN). (The employee’s supervisor may submit the form on behalf of the employee if the employee is hospitalized one (1) night or incapacitated or unavailable for more than three (3) consecutive days.)
  2. A Certification of Health Care Provider

For expectant mothers: WH-380-E Certification of Health Care Provider for Employee’s Serious Health Condition (link requires VPN)

For expectant fathersWH-380-F Certification of Health Care Provider for Family Member’s Serious Health Condition (link requires VPN) – OR – A doctor’s statement on letterhead with the expected delivery date and the beginning and ending dates for the period of incapacitation.

After the birth of the child, submit one of the following:

  • Doctor’s statement on letterhead confirming the date of the child’s birth
  • Verification of birth facts
  • The child’s Birth Certificate

Employees ineligible for leave for the birth of a child under the FMLA may be eligible for leave under Parental Leave.
