Military Exigencies Leave
Qualifying exigencies covered under the FMLA:
- Short-notice deployment
- Military events and related activities
- Child care and school activities
- Financial and legal arrangements
- Counseling
- Rest and recuperation
- Parental care
How to Apply
Submit ALL items below via LeaveAdministration@UTSouthwestern.edu to Leave Administration for consideration of the FMLA request.
- A Family Medical Leave Request. (The employee’s supervisor may submit a request form on behalf of the employee if the employee is unavailable for more than three (3) consecutive days.)
- A completed Certification of Qualifying Exigency for Military Family Leave
- A copy of the military member’s active-duty orders (or notification of an impending call to active duty) for the related deployment.
- Certification providing the appropriate facts supporting the related qualifying exigency for which leave is sought.
Some examples are:
- To meet with an alternative child care provider or a financial consultant. (The contact information of the third party whom the employee is meeting may be required.)
- To spend time with a military member on Rest and Recuperation leave. (A copy of the military member’s Rest and Recuperation orders or other documentation confirming the dates of the member’s leave is required.)
