Total Care Program
The Total Care Program is a patient-centered initiative committed to the comprehensive care and management of Parkinson's disease, aiming to empower patients and caregivers and to improve their quality of life.
What we do
We are focused on three key approaches to provide comprehensive care for our patients and care partners
- Co-design: Engaging patients and care partners to prioritize program focus based on their needs.
- Personalized care: Connecting patients and families with support and resources considering their disease progression and psychosocial concerns.
- Holistic care: Streamlined access to diverse supportive care elements, including support groups, medical family therapy, ancillary and palliative care, and comprehensive education.
Collaborations
In collaboration with the Peter O’Donnell Brain Institute, our expert team comprises movement disorders neurologists, neurosurgeons, palliative care, neuropsychology, PMR, physical therapy, occupational therapy, speech therapy, and social workers. Together, we strive to deliver excellent clinical care for patients with Parkinson's disease.
