Gift by Donor’s Agent or Guardian before Donor’s Death
You may donate your loved one to the Willed Body Program prior to death if you are the Donor’s Agent or Guardian by completing the agreement packet in the next section and submitting a copy of the relevant court documents.
Agreement for Donation to the Willed Body Program
For Donation by agent or guardian, only sections 1, 2, 3, & 5 should be completed.
*Please note for Donors Agents: If you are the Donor’s “agent”, then a copy of the medical power of attorney or other record signed by the Donor must be attached.
*Please note for guardian: If you are the legal guardian of the Donor, then a copy of the relevant court order must be attached.

At Need Donations
The Willed Body Program can also accept donations of persons not previously registered after death has occurred. Acceptance is determined on a case-by-case basis.
