Verification of Faculty Employment and Privileges

Verification of employment for current and past faculty members can be requested by contacting The Work Number, a service of Equifax, and entering UT Southwestern's Employer Code: 13946.

Verification of privileges at hospitals affiliated with UT Southwestern can be obtained through the Medical Staff/Allied Health Staff Verification website.

Verification of Graduate Medical Education Training

To request verification of residency or fellowship training, please complete the Verification Request Form.

Peer Reference Requests

There is no charge for Peer Reference requests that exclude training dates.

Elective and Rotator Requests

Program Coordinators and Administrators can use the Elective and Rotator Requests form to request rotations for residents or fellows on a UTSW Radiology Service. The form accommodates both internal and external rotators, and a team member from the Radiology Education Office will follow up with you within 30 days of submission.

Rotation requests for medical students are not permitted through this route. Please review our Medical Student Education website and reach out to the listed coordinator.
