Patient Notification

UT Southwestern Medical Center has sent notifications to individuals whose data may have been inappropriately accessed. On Aug. 12, 2024, UTSW learned that an unauthorized individual gained access to patient registration information at our Frisco medical office building. Any incident that jeopardizes privacy is taken very seriously, and we want to assure you that it is being properly investigated and appropriate remedial actions have taken place in accordance with UTSW policies and regulations. Credit monitoring is being offered to all impacted individuals. Information including name, address, date of birth, Social Security number, driver’s license number, and financial, medical, and health insurance details could have been accessed impermissibly, although we are not aware of improper use of data.
