What to do if ...
- I have symptoms.
If you are symptomatic, before contacting Occupational Health, the first step for employees is to:
- I have traveled.
Employees must comply with any international, federal, or state testing or quarantine requirements following travel to specific locations.
- I am an employee who has been exposed outside of UTSW.
If you are symptomatic, before contacting Occupational Health, the first step for employees is to:

If you have been exposed to COVID-19 at UTSW, please fill out the Electronic First Report of Injury form.

Please fill out and submit the Multiple Mass Exposure form.
- I have questions about returning to work
Please refer to the grid. Upon completion the screening form, a nurse will advise you about returning to work.
- I have questions about required immunizations.
The HSO-204: Vaccine-Preventable Diseases Policy mandates that all clinical employees document proof of immunity to certain communicable diseases, including measles, rubella (German measles), mumps, and varicella (chicken pox). If proof of immunity is not available, blood can be drawn to assess immune status or vaccine can be given.
Influenza immunizations are offered to all employees in the fall. Employees are strongly encouraged to take advantage of this opportunity as a means of protecting themselves, their families, and patients from influenza. If the employee is covered by UT Select insurance, Occupational Health will file it under their insurance.

Please note, children under the age of 18 are not permitted to attend your appointment or wait in the waiting area due to the risk of exposure. Your appointment will be rescheduled.

Answers to FAQs
- Occupational Health clearance is not needed for travel.
- Any COVID-19-related employee exposures in the workplace should be reported immediately to Occupational Health by submitting the electronic Multiple Exposure Reporting Form.
- An exposure risk assessment will be conducted for each employee identified on the multiple exposure form by Occupational Health staff.
- Recommendations regarding work restrictions, symptom monitoring, and/or testing will be determined by the Infectious Disease team and Occupational Health.
- Any department communications regarding work restrictions, symptom monitoring, and/or testing must be approved by Incident Command.
