It is a pleasure to inform you of your appointment as a Postdoctoral Resident/Fellow in the Division of Child and Adolescent Psychiatry of the Department of Psychiatry at University of Texas Southwestern Medical School (UT Southwestern) for the period of _______________through ________________. Your program will be under the direction of Dr. Maryam Rezai. Your appointment will be one year in duration.
All residents and fellows are required to undergo a mandatory substance abuse test and criminal background check at Children’s Medical Center of Dallas (Children’s), your primary training facility. All appointments are contingent on successful completion of all Children’s credentialing requirements
Reappointments are based on evaluation by program faculty, recommendation to Children’s Medical Center (Children’s) by the program director, and successful completion of all annual Children’s credentialing requirements. You will be notified not less than three months prior to the conclusion of this appointment if the faculty from your program does not intend to offer an appointment to you for the following year.
This fellowship shall be for the education and training of the recipient and no employee-employer relationship shall be allowed to exist therein. As a condition for receipt of this award, no service shall be performed.
Stipend
This award provides a (PGY) training stipend of $___________ for postdoctoral trainees commensurate with your postgraduate experience (i.e. years since you received your MD or PhD). UT Southwestern University training stipends are not subject to federal taxes, deductions and withholdings. The fellow will be responsible for paying his/her individual taxes.
Health Care Benefits
We will also provide reimbursement funds to UT Southwestern specifically for the cost of medical, dental and vision care for you and any members of your immediate family, based on the rates set by UT Select, Delta Standard, and Superior Vision, respectively; you may supplement these funds to obtain an alternative plan of your choice, but the funds for health are not redeemable for other purposes.
Work Hours
Work hours of the House Staff shall be in accordance with the training programs standards set forth by UT Southwestern.
Parking
Children’s shall provide parking, at no cost, to the resident/fellow while training at the facility.
Meals
Call Meals are available to residents/fellows as outlined in Children’s Call Meal Policy. This policy can be found on Children’s intranet web site Childnet.
Licensure/Permits
To treat patients at Children’s, House Staff must have either a Texas License or Physician in Training Permit. The House Staff agrees he/she will maintain his/her license in accordance with the laws and regulations of the State of Texas. Children’s will reimburse the actual cost of Physician in Training permit only after a copy of the receipt and issued permit has been submitted to Children’s. Full Texas Licensure costs are not reimbursed by Children’s. House Staff are responsible for notifying Children’s of any lapse in licensure. The House Staff privileges at Children’s are subject to immediate suspension if these requirements are not met.
Immunization
The House Staff agrees to secure immunizations for measles, diphtheria, tetanus, mumps, rubella, and hepatitis B, submit to a yearly tuberculosis test, and to keep such immunizations current during the term of this agreement and provide written documentation. Arrangements for immunizations and testing shall be made by and at the direction of the Children’s Employee Health Department. Failure to comply with this provision will result in suspension of privileges unless a waiver has been obtained from Employee Health.
Medical Liability
Professional liability coverage is provided through the University of Texas System Professional Medical Liability Benefit Plan. A copy will be provided to you upon request by Children’s Medical Center of Dallas.
Medical Records
It is agreed and understood that failure to complete medical records according to rules and regulations of Children’s or affiliated hospitals will result in immediate suspension. Suspension shall remain in effect until all documentation is completed.
Safety
The House Staff agrees to abide by Children’s Infection Control, Quality Improvement, Risk Management and Occupational Health Policy and Procedures. House Staff agrees to report adverse patient occurrence or outcomes in accordance with Children’s Polices and Procedures.
Financial Influences / Relationships
A House Staff may never accept anything from a drug company or related entity in exchange for prescribing products or for a commitment to continue prescribing products.
House Staff are never to accept grants, scholarships, subsidies, support, gifts, consulting contracts, cash or cash equivalents, educational or practice related items, meals, entertainment or recreation, or items of personal benefit from a drug company. Items not of substantial value ($100 or less), intended for the benefit of patients may occasionally be accepted but must be reported to the Children’s GME Department. An occasional modest meal may accompany a scientific or educational presentation provided that the venue and manner of presentation is conductive to scientific or educational interchange.
A House Staff, family member or associated entity may not have equity interest or yearly income of $10,000 or more than 5% or more ownership interest in any business related to drug manufactures.
Cooperation with Investigations
House Staffs agree to cooperate fully with Children’s Risk Management staff or legal counsel in investigating any claim seeking damages by a third party that may be asserted against the House Staff or Children’s based upon the conduct of the House Staff or other Children’s employees. In the event the House Staff is served with citation in a lawsuit arising out of conduct at Children’s, he/she must immediately report same to Director of Legal Affairs.
Certifications
If the House Staff fulfills his/her tenure of obligation and discharges his/her duties to the entire satisfaction of UT Southwestern and Children’s, Children’s will issue a certificate signed by the UT Southwestern Department Chairman and the President of Children’s indicating satisfactory performance of said duties. Children’s specifically reserves the right to withhold the aforesaid certificate of recognition until all rules, regulations and personnel polices have been complied with, including but not limited to payment of any and all money due to Children’s and completion of all medical records in accordance with the rules and regulations of Children’s Medical/Dental Staff Bylaws.
If House Staff fails to remain on the service the full time of his/her agreement, he/she forfeits the right of obtaining any certificate of recognition for the time served at Children’s and the incident will be reported to the American Medical Association Council on Medical Education, the Accreditation Council on Graduate Medical Education, and the Texas Medical Board.

It is recognized that in addition to the consideration set forth above, you will receive the benefits of practicing medicine in an academic setting, which will include community good will from association with the University, exposure to training or knowledge (some of which may be proprietary) not otherwise immediately accessible to the general medical public, and which may also include the expenditure of public funds on your behalf.
Expectations
The House Staff agrees to provide appropriate and conscientious care to his/her patients and to abide by the published Medical/Dental Staff Bylaws.
We anticipate that this training is part of your preparation for a career in which you will serve as consultant, a teacher, and a contributor to the scientific literature in your subspecialty. We intend to offer an experience designed to prepare you cognitively and technically to practice your pediatric subspecialty and to train others in the discipline. The goal of our clinical training is to produce fellows who not only have a comprehensive fund of knowledge in their subspecialty, but who are also able to think critically about a clinical situation as well as being exemplary teachers of the art and science of medicine.
Towards these ends, we will require you to attend certain programs, which will be enumerated by your Program Director, that are essential for your postdoctoral education. We hope you will also attend many of the seminars, lectures, and conferences that are part of the enriching environment of UT Southwestern and Children’s.
Support for Scholarly Activity
An essential component of your fellowship is the development of a scholarly product under the guidance and direction of a group of faculty invested in and responsible for your education. This group will be organized by your program director to meet specific needs of your career.
